A meningococcal booster shot (MCV4) is required for all 11th graders on their 16th birthday.
DUE DATE on or before September 2, 2025 for students that have turned 16 years old.
Your child will not be allowed to attend school after the due date, (9/2/2025) until a valid and up to date GA immunization Form 3231 is received at the school. If your child has yet to turn 16 years old, you will have a 30 day grace period to get the vaccination and provide the school with an updated Form 3231.
To assist parents, the Columbia County Public Health Department (706-868-3330) is offering a walk-in clinic every Wednesday for vaccinations. Or, you can visit your provider of choice.
